Family membership consists of up to four swimmers, either one or two parents and up to two or three school age children, or up to four school age children. Ages are as at race day, January 3, 2010. This form must be lodged by midnight, Friday, January 1, 2010. This form will not be accepted unless a responsible parent has already entered and paid online for family entry. Registration id number for Family Entry must be included below.
